Parent Guardian Permission


By signing this document, I, , am giving permission for  to participate in the High School Bigs program.

  1. My child may participate as a mentor until I notify Big Brothers Big Sisters that he/ she cannot participate. Big Brothers Big Sisters of WNC reserves the right to not match a prospective volunteer and to terminate a volunteer’s service if the program deems that the volunteer is not fulfilling his/her commitment.
  2. I give permission for my child to participate in the Big Brothers Big Sisters enrollment process (brief interview, 2 references, training).
  3. I understand that transportation to the after-school site is the responsibility of my child.
  4. I understand that no contact between my child and his/her assigned student is allowed outside of the after-school setting except by mail or phone calls.
  5. I give permission for my child’s photo and first name to be used for marketing purposes.
  6. This permission form serves as a positive reference for my son/ daughter to participate in this program as a mentor.
